Community Health Systems is seeking a Patient Navigator RN for the Pulmonology Clinic in Tucson, AZ. The role guides patients and families across the continuum of care, coordinating services and education to support treatment adherence and outcomes.

The RN will collaborate with interdisciplinary teams, assist with insurance matters, and develop patient education materials while maintaining compassionate, patient-centered care.

Qualifications

• 3-5 years of clinical nursing experience required.
• 1-3 years of experience in patient navigation, care coordination, or case management preferred.

Responsibilities

• Serves as primary point of contact for patients, families, and caregivers from diagnosis through end of treatment.
• Assesses patients' medical, emotional, and socioeconomic needs, addressing barriers to care and coordinating solutions.
