Job summary

Jobtailor is seeking a registered nurse to act as a consistent care coordinator for adults with complex diseases. You will liaise with physicians, patients, and families while coordinating appointments and access to services.

In this role you will educate patients, provide care coordination, referrals, and support, and work with a multidisciplinary team to improve outcomes and transitions of care.

Qualifications

• Must hold active RN licensure in PA or multi-state license via eNLC.
• 1–3 years of nursing experience in the specialty.
• Current CPR certification from the American Heart Association.
• Compliance with HIPAA and data security guidelines.

Responsibilities

• Serve as primary contact for physicians, patients, caregivers, and families.
• Coordinate patient flow by arranging appointments and access to services.
• Manage care coordination for adults with specialized diseases with the team.
• Educate patients, provide support, referrals, and resources as needed.
• Oversee community outreach and engagement with high-risk populations.
• Coordinate transitions of care between inpatient and outpatient settings.
