St. Mary's Medical Center in West Palm Beach seeks a Navigator for the SMMT Grant to support clients during pregnancy and up to one year postpartum. The role coordinates subspecialty appointments, conducts community outreach, and acts as the primary contact for clients and referring providers.

The navigator maintains patient files, prepares reports, coordinates meetings, handles calls with professional etiquette, and ensures seamless coordination between community resources and patients.

Qualifications

• 2-3 years of nursing experience.
• RN license with BLS/CPR certification.
• Associate degree or Bachelor's degree preferred.

Responsabilités

• Update and maintain client files and reports.
