Job summary

Atrium Health Wake Forest Baptist is seeking an experienced RN Patient Navigator to guide patients through the healthcare system, coordinating care from diagnosis to survivorship. This role requires BSN, NC RN licensure, and 2+ years of nursing experience; 3-5 years preferred, with strong communication and care management skills.

The Navigator will work 40 hours per week at the Wake Forest Baptist Medical Center, with potential travel to other Wake locations, and will participate in program

Qualifications

• BSN from an accredited program.
• Two years of RN experience in the required specialty.
• NC RN licensure; BCLS certification preferred.
• RN certification preferred.

Responsibilities

• Guide patients through the healthcare system from diagnosis through survivorship.
• Identify and address barriers to timely care and coordinate services.
• Assist with transitions from active treatment to ongoing care.
• Document outcomes and participate in program evaluation.
• Engage with community providers to support prevention and early detection.
