Atrium Health Wake Forest Baptist is seeking an experienced RN Navigator to guide patients through the healthcare system, coordinating care from diagnosis through survivorship. The role emphasizes patient‑family centered care, community outreach, and collaboration with the healthcare team.

The RN Navigator works within Home Health Nursing Services, ensuring effective communication, documentation, and adherence to program standards while supporting age-specific patient care and continuous quality

Qualifications

• Graduate from an accredited school of nursing, BSN required.
• Minimum two years' experience as a registered nurse in the specialty required.
• Current licensure to practice as Registered Nurse in applicable state. Certification as an RN preferred. BCLS required.

Responsibilities

• Utilizes the nursing process to provide patient/family centered care.
• Coordinates the effective and efficient delivery of patient care services.
• Documents in accordance with departmental reporting standards and maintains program data.
• Implements outreach to establish and maintain relationships with community agencies.
• Demonstrates professional practice and participates in governance, standards of care and professional development.
• Communicates effectively to promote information sharing across the continuum of care.
• Responsible for ongoing evaluation and quality improvement of the Navigator Program and tracks activity.
• Works with community-based service providers to address community needs for services and education.
• Provides age-appropriate patient care following age-specific guidelines.
