Responsibilities

• Guides patients and families through their healthcare experience, answers questions, provides education and helps them understand their diagnosis and treatment options.
• Follows patients from date of diagnosis through care-path completion to ensure all options are explored.
• Reviews patient history and updates medical records in a timely manner.
• Collaborates with other health care team members and communicates workflow concerns to leadership.
• Assists with scheduling and pre-procedure clearances as needed.
• Maintains professional knowledge through workshops and professional activities.
