Job summary

Ascension is seeking a Registered Nurse Patient Navigator to guide patients and families through complex care journeys within our Healthcare & Medical Services team. In this compassionate, patient-focused role, you will coordinate care across multiple providers and settings, support transitions, and remove barriers to timely treatment.

You’ll perform clinical assessments, educate patients, and connect them with resources while advocating for their needs.
