Job summary

The-Urology-Group is hiring a Patient Navigator RN to assist in assessing, planning, implementing, evaluating, and documenting nursing care in accordance with facility and professional practice. You will help patients understand their diagnosis, coordinate treatment plans, and communicate with providers and staff.

Requires graduation from an accredited nursing program, an active RN license, and at least 2 years of clinical experience.

Qualifications

• Graduate of an accredited nursing program is required.
• BSN preferred.
• RN License and state certification required.
• BLS certification required.

Responsibilities

• Guides patients and families through their healthcare experience and treatment options.
• Follows patients from diagnosis through completion of the care-path.
• Reviews patient history and updates medical records in a timely manner.
• Coordinates with the surgical scheduling department for patient prep and clearances.
• Maintains professional knowledge through workshops and publications.
