Job summary

MidLantic Urology in Raleigh, NC is seeking a Patient Navigator RN to guide patients through diagnosis, treatment options, and care coordination. You will work with providers and staff to plan and document nursing care, and help patients understand their paths across multiple treatment options.

Requirements include a graduate of an accredited nursing program with 2 years of clinical experience; RN license and BLS certification; BSN preferred.

Qualifications

• Graduate of an accredited nursing program required. BSN preferred.
• 2 years' clinical experience.
• RN license and BLS certification required.
• Knowledge of EMR systems and HIPAA compliance.

Responsibilities

• Guides patients and families through their healthcare experience, answers questions, provides education and helps understanding of diagnosis and treatment options.
• Follows patients from diagnosis through care-path to ensure all options are explored.
• Reviews patient history and updates medical records in a timely manner.
• Communicates problems with workflow or procedures to the manager or director of clinical services.
• Collaborates with surgical scheduling regarding patient preparation and pretesting results.
