Job summary

Banner Health is seeking an RN Patient Navigator to provide inpatient and outpatient care coordination for Advanced Lung Disease patients at the BUMG Advanced Lung Disease Clinic in Phoenix, AZ. The role focuses on comprehensive care coordination, patient education, and liaison work with physicians and caregivers.

Ideal candidates will have an active RN license, BSN or higher, and 5 years of clinical experience with strong organizational and communication skills.

Qualifications

• Bachelor's degree in nursing and RN licensure required.
• Current BLS certification.
• Strong organizational and communication skills.

Responsibilities

• Assesses the patient’s plan of care and coordinates resources to meet health needs.
• Evaluates medical necessity and coordinates tests and consultations.
• Guides patients and families through diagnostic services and care planning.
• Ensures patient and family understand care processes and next steps.
• Collaborates with multidisciplinary team to optimize outcomes across care settings.
• Supports end-of-life discussions and care planning when appropriate.
