Job summary

Banner Health is seeking a dedicated RN Case Manager in our Phoenix, AZ area to provide comprehensive care coordination and clinical case management for assigned patients. You will assess needs, develop individualized care plans, coordinate resources, monitor progress, and ensure seamless care across the healthcare continuum.

This role collaborates with providers, patients, employers, and payers to promote safe, effective, and cost-efficient care.

Qualifications

• Bachelor's degree in case management or health care.
• Current Registered Nurse (RN) license in state worked.
• BLS certification required for acute care assignments.
• 3-5 years of clinical experience in case management or utilization review.
• Working knowledge of care management and resource/utilization management.
• Ability to work effectively in an interdisciplinary team.
• Flexibility to work rotating hours and on-call when needed.
• Experience in acute care and/or home care settings is preferred for Banner Registry/Travel positions.

Responsibilities

• Assess patient healthcare needs and evaluate plans of care to identify opportunities for intervention and support.
• Develop, implement, monitor, and update individualized care coordination plans based on patient needs and clinical goals.
• Coordinate care across the continuum by facilitating communication among patients, providers, employers, payers, and ancillary services.
• Monitor patient progress and evaluate the effectiveness of care plans and interventions.
• Identify barriers to care and develop strategies to improve patient outcomes and access to services.
• Facilitate referrals to appropriate healthcare services, specialists, community resources, and support programs.
• Educate patients and families regarding treatment plans and care transitions.
