Job summary

Qualifications

• Bachelor's degree required.
• Registered Nurse license required.
• American Heart Association – Basic Life Support certificate required.
• 3-5 years related clinical medical-surgical experience required.
• Strong analytic skills with quality and data analysis.
• Excellent interpersonal skills.
• Experience with interdisciplinary health care approach.
• Experience with computer systems including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint, Access).
• Preferred: OCN, Patient Navigator or Nurse Navigator certification.

Responsibilities

• Initiates communication with patients after positive findings and guides them through the health care system.
• Acts as liaison between patient, PCP, surgeon, medical oncologist and other providers.
• Facilitates interactions among health care staff to improve coordination of care.
• Provides health education to individuals and groups; maintains patient education library.
• Identifies involved departments and builds relationships with providers.
• Assists with questions about payment for services; coordinates transportation when needed.
• Builds relationships with other patient navigators and tracks interventions and outcomes.
• Participates in medication reconciliation during transitions of care.
