Responsibilities

• Initiates communication with patients after positive findings and guides them through the health care system.
• Acts as liaison between patient, PCP, surgeon, oncologist, and other providers.
• Facilitates interaction among health care staff and providers.
• Provides health education to individuals and groups and maintains educational materials.
• Identifies involved personnel and builds relationships to coordinate care.
• Manages questions about payment and arranges patient transportation when needed.
