Job summary

Memorial Healthcare System seeks a dedicated nurse/advocate to guide patients and families through the continuum of care. You will liaise among patients, families, clinicians, and community resources to ensure access to appropriate services and timely information.

Qualifications

• Accredited nursing program or equivalent.
• BLS certification required.
• Florida RN license required; Radiation Therapy license if in Radiation Oncology (ARRT).
• Master's in Social Work (MSW) preferred if applicable.

Responsibilities

• Continuously reviews patient progress as per the plan of care.
• Communicates plan of care to patient and family; solicits concerns for resolution.
• Assist patients and families with financial, psychosocial, functional, and administrative issues; refer to resources.
• Coordinate care with clinical staff and community resources to maximize plan outcome and resolve barriers.
• Provide emotional support, counseling, and education to patients and families to aid decision-making.
