Job summary

CoxHealth is seeking a Care Transitions Navigator for CoxHealth South in Springfield, MO. The role focuses on coordinating transitions of care, liaising with patients, clinics, and the Prior Authorization Team within the 340B contract pharmacy South department.

Responsibilities include facilitating home care services, assisting with medication assistance programs, and ensuring smooth patient handoffs between facilities. MO RN license required; nursing program graduate."

Qualifications

• Graduate nursing program required.
• 1 year nursing experience preferred.
• 6 months home health preferred.
• Knowledge of Medicare/Medicaid regulations desirable.

Job Description

The Care Transitions Navigator maintains ongoing contact with health care organization, Insurance companies, and medical community to facilitate home care services, Specialty Medications, financial assistance programs for medication assistance, and streamlines the transition of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator will work as a liaison between patients, clinic staff and the Prior Authorization Team. This role may involve occupational exposure to hazardous drugs. Training and education will be provided on any specific procedures, risks and safety protocols associated to the handling of these drugs.

Education

• Required: Graduate of an approved nursing program

Experience

• Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.
• Preferred: Six (6) months home health experience.

Skills

• Knowledge of Medicare and Medicaid regulations desirable.

Licensure/Certification/Registration

• Required: MO RN License
