Job summary

CoxHealth is seeking a Care Transitions Navigator to coordinate patient transitions across facilities and care teams. This role involves liaising with patients, clinic staff, and the Prior Authorization Team to facilitate home care services, medications, and financial assistance programs.

Qualified candidates are graduates of an approved nursing program with MO RN licensure and ideally one year of nursing experience; home health exposure is a plus.

Qualifications

• Knowledge of Medicare and Medicaid regulations desirable.

Responsibilities

• Maintain ongoing contact with healthcare organizations, insurance companies, and the medical community to facilitate home care services, specialty medications, and financial assistance programs for medication assistance.
• Serve as liaison between patients, clinic staff, and the Prior Authorization Team to streamline transitions between facilities and care providers within CoxHealth 340B clinics.
• Support safety protocols for handling hazardous drugs; training provided on procedures, risks, and safety measures.
