Job summary

CoxHealth South in Springfield, MO is seeking a Care Transitions Navigator to liaise between patients, clinics, insurers, and the Prior Authorization Team. This role focuses on home care services, specialty medications, and financial assistance programs within CoxHealth 340B clinics.

The role involves coordinating transitions between facilities, ensuring safety with hazardous drugs and completing required training. Office hours are 8:00am–4:30pm, Day Shift.

Qualifications

• Graduate of an approved nursing program.
• MO RN License is required.
• Preferred: 1 year of professional nurse experience in the last 3 years; 6 months home health preferred.

Responsibilities

• The Care Transitions Navigator liaises between patients, clinic staff and the Prior Authorization Team to facilitate home care services and medications.
• Coordinates with insurance companies and medical community on medication assistance and patient transitions between facilities.
• May involve exposure to hazardous drugs; training and safety protocols provided.

The Care Transitions Navigator maintains ongoing contact with health care organization, Insurance companies, and medical community to facilitate home care services, Specialty Medications, financial assistance programs for medication assistance, and streamlines the transition of patient care between facilities and care providers within CoxHealth 340B clinics. The Navigator will work as a liaison between patients, clinic staff and the Prior Authorization Team.

This role may involve occupational exposure to hazardous drugs. Training and education will be provided on any specific procedures, risks and safety protocols associated to the handling of these drugs.

Education

• Required: Graduate of an approved nursing program

Experience

• Preferred: Minimum one (1) year experience as professional nurse within the last three (3) years.
• Preferred: Six (6) months home health experience.

Skills

• Knowledge of Medicare and Medicaid regulations desirable.

Licensure/Certification/Registration

• Required: MO RN License
