Valeris is seeking a Patient Journey Lead-RN to support the program team in managing people, processes, and systems within the HUB and Patient Assistance Program. This full-time role involves monitoring high-volume patient journeys, acting as a navigator for program access or affordability, and delivering detailed information to stakeholders.

The ideal candidate holds an RN license with strong data analysis, communication, and MS Office skills, plus the ability to interpret data, plan, and meet

Competenze

• Registered Nurse license required or equivalent.
• Bachelor's degree preferred; or equivalent experience considered.

Mansioni

• Monitor patient journey process in a high volume HUB.
• Advocate and navigate for patients; coordinate information with stakeholders.
• Analyze data and processes to improve program efficiency.
• Develop and audit program processes to ensure adherence.
• Provide operational reporting to the client to gauge program health.
• Identify trends and work with stakeholders to implement corrective actions.
• Support training and follow SOPs; ensure confidentiality.

Formazione

RN license
