Job summary

Winchester Hospital is seeking compassionate Registered Nurses to join the Patient Flow, Admissions & Discharge RN Team. The role focuses on coordinating admissions and discharges across hospital areas, helping families navigate the hospital experience with confidence and clarity.

Ideal candidates will have MA RN licensure and at least two years of clinical nursing experience, with BSN preferred. The position supports patient throughput initiatives and collaboration with multidisciplinary teams.

Qualifications

• Graduate of an accredited or state-approved Registered Nursing program.
• Massachusetts RN license is required.
• Two years of clinical nursing experience.
• BSN degree preferred.
• Experience in Medical-Surgical Nursing Care Coordination Transitions of Care Patient Flow or Throughput Operations.

Responsibilities

• Complete comprehensive patient admission assessments and documentation.
• Perform medication reconciliation and identify potential medication-related concerns.
• Coordinate and facilitate timely patient admissions and discharges.
• Provide individualized patient and family education regarding hospitalization, care plans, and discharge instructions.
• Collaborate closely with physicians, case managers, nursing staff, and ancillary departments to support effective care transitions.
• Advance patient throughput initiatives and care coordination efforts.
• Help reduce administrative demands on bedside nursing teams, creating greater capacity for direct patient care.
• Promote a culture of safety, efficiency, and patient-centered excellence throughout the hospitalization journey.
