Qualifications

• Specialized Diploma Graduate of an accredited school of nursing.
• Knowledge of patient- and family-centered practice and collaboration with patients, families, and care partners.
• Cultural awareness and respect for diverse backgrounds.
• Effective communication and delegation within the care team.
• Knowledge of patient assessment and care planning.
• Knowledge of medication administration and prescription considerations.
• Discharge planning and arranging follow-up care.
• Documentation of health information in written or electronic form.
• BLS certification (AHA) and active RN license in PA.
• Ability to work with inter-professional teams and in diverse settings.

Responsibilities

• Interacts with patients, families, and staff to deliver optimal healthcare.
• Identifies learning needs and educates patients and families on conditions, treatments, and medications.
• Uses information technology to communicate, monitor outcomes, and support ethical decisions.
• Works within nursing and inter-professional teams with open communication and shared decision-making.
• Creates an inclusive environment; pursues professional growth and acts as a preceptor when appropriate.
• Participates in safety and quality improvement committees and incorporates feedback.
