Job Description

Summary

The Home Health Patient Care Manager/Registered Nurse is responsible for supervising and coordinating clinical services, overseeing an interdisciplinary team to ensure high-quality care in accordance with physician plans and legal regulations.

Responsibilities

• Handle referrals, assign clinicians/therapists, and authorize start of care visits.
• Coordinate patient benefits, medical necessity, and insurance approvals.
• Assess patient needs continually, ensure care is individualized and meets physician orders.
• Develop and update patient care plans.
• Communicate with physicians, clinicians, patients, and other healthcare providers; document all interactions.
• Review assessments and care plans daily, providing recommendations as needed.
• Coordinate care across disciplines and providers.
• Follow up on tests, physician contact, and changes in patient condition; communicate updates.
• Facilitate case conferences and ensure effective information exchange.
• Assist with patient transfers and discharges.
• Manage orders, visit schedules, and documentation of verbal orders.
• Ensure timely and accurate documentation and review clinician notes weekly.
• Supervise service delivery, conduct patient visits, and participate in quality programs.
• Report incidents, assist in onboarding new staff, and provide team leadership.
• Provide direct patient care when necessary and participate in on-call rotations.
• Complete required training and participate in episode management.
• Perform other duties as assigned.

Education and Experience

• Current RN license in practice state.
• CPR certification.
• Valid driver's license, vehicle insurance, and reliable transportation.
• At least one year of clinical experience in Louisiana; RN licensure must be unrestricted.
