Job Description

Summary

The Home Health Patient Care Manager/Registered Nurse is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team to ensure high-quality care to home health patients in accordance with physician plans and all relevant laws and regulations.

Responsibilities

1. Receive referrals, assign clinicians/therapists, and authorize start of care visits.
2. Determine patient home health benefits, medical necessity, and insurance approvals.
3. Assess patient needs continuously and ensure care is individualized, appropriate, and complies with physician orders.
4. Develop, implement, and update the patient plan of care.
5. Manage communication with physicians, clinicians, patients, and referral sources, documenting interactions appropriately.
6. Review assessments and care plans daily, providing recommendations to clinicians.
7. Coordinate care with all involved disciplines and providers.
8. Follow up on lab tests, diagnostic results, and changes in patient condition, ensuring physician notification and plan adjustments.
9. Facilitate case conferences and reports, ensuring effective interdisciplinary communication.
10. Assist with patient transfers and discharges as directed by physicians.
11. Receive patient status reports from field clinicians and process orders accordingly.
12. Ensure timely and accurate documentation of clinical activities and correct any adverse findings.
13. Supervise service delivery quality and utilization through reviews and visits.
14. Document incidents in the Risk Management system as required.
15. Assist in onboarding new personnel and provide leadership to the clinical team.
16. Participate in QAPI programs and on-call rotations.
17. Ensure compliance with clinical and coding standards and participate in the Episode Management process.
18. Complete required training and regulatory courses.
19. Perform other duties as assigned.

Education and Experience

• Current RN licensure in Louisiana.
• CPR certification required.
• Valid driver’s license, vehicle insurance, and reliable transportation.
• Minimum one year of clinical experience in Louisiana with unrestricted RN licensure.
