The Registered Nurse in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws and policies. The RN coordinates the care plan with healthcare professionals and ensures continuity of care through effective communication with physicians, patients, families, and community resources, all within a Point of Care setting.

Responsibilities

1. Clinical Competence

• Develops and revises the care plan in collaboration with physicians and healthcare team.
• Supervises care provided by aides and LPNs, provides instruction, and assigns tasks according to regulations. Conducts supervisory visits as required.

2. Documentation and Care Delivery

• Delivers high-quality clinical services within scope, adhering to infection control standards and the plan of care.
• Completes assessments (OASIS) accurately and timely, including medication reconciliation.
• Documents patient visits thoroughly and promptly, complying with policies and payer requirements.

3. Quality

• Performs initial and comprehensive nursing evaluations, ensuring patients meet eligibility and medical necessity guidelines.
• Communicates effectively with staff, physicians, patients, and families to coordinate care and ensure quality service.

4. Professionalism

• Arrives punctually, participates actively in meetings, and maintains a positive attitude.
• Completes training and complies with all policies.

5. Teamwork

• Takes direction from management, completes tasks timely, and participates in orientation and performance improvement activities.
• Participates in on-call and weekend rotations as needed.

Education and Experience

License Requirements

• Current RN licensure in state of practice
• Current CPR certification
• Valid driver's license, vehicle insurance, and access to transportation

Additional State Requirements

• LA: Minimum of one-year clinical RN experience; this may be waived if recent LPN experience is held. RN licensure must have no restrictions.
