Responsibilities

Clinical Competence

1. Provide high-quality clinical services within the scope of practice, following the plan of care, and in coordination with the healthcare team, adhering to infection control procedures. Do not alter the plan of care.
2. Monitor and report patient needs to the supervising RN.

Documentation and Care Delivery

1. Communicate effectively with physicians, patients, families, and staff regarding patient care, visits, schedules, and documentation, including OASIS data sets and coding.
2. Document patient visits thoroughly, maintaining accurate medication lists and complying with policies and payer requirements.

Quality

1. Participate in performance improvement initiatives to reduce hospitalizations and improve patient outcomes.

Professionalism

1. Attend all meetings punctually, participate actively, and maintain a positive attitude.
2. Complete required training and comply with policies.

Teamwork

1. Participate in the agency’s Episode Management process.
2. Perform other duties as assigned.

Education and Experience

License Requirements:

• Current LPN licensure in Louisiana without restrictions.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.
• CPR certification.

Additional State-Specific Requirements:

• Minimum of one year of experience as an LPN.
