Responsibilities

Assess the physical, psychological, and social dimensions and educational needs of the patient on admission.
Identify patient needs and formulate a written care plan within 24 hours of admission.
Implement plan of care and document effectiveness; update as necessary.
Provide patient/family education and support to achieve independence in care.
Adhere to infection-control policies and nursing standards; ensure safety and quality of care.
Document actions and communicate patient status to the team; participate in problem-solving.
