ProHealth Physicians, part of the Optum family, is seeking a registered nurse to support the care team and patients by providing and documenting nursing care following established standards and practices. This role requires CT licensure, 1+ year of nursing experience, and familiarity with Epic; it involves patient education, coordinating services, and accurate EHR documentation.

Formación

• Active, unrestricted RN licensure in CT.
• 1+ year of nursing experience.
• Intermediate level of computer proficiency in Microsoft Office.

Responsabilidades

• Provide and document patient care such as injections and screenings.

• Educate patients about illness and self-management.
• Document interactions accurately in the EHR.
• Coordinate care with regional management and care team.
