Job summary

ProHealth Physicians, part of the Optum family within UnitedHealth Group, seeks an experienced registered nurse for a full-time role in Vernon, CT. You will provide direct patient care, perform injections and screenings, and document care in the electronic health record.

Qualifications

• Active, unrestricted RN license in CT.
• 1+ years of nursing experience.
• Intermediate level of computer proficiency in Microsoft Office.

Responsibilities

• Provide and document patient care such as injections and preventive screenings.
• Triage patients according to approved protocols.
• Prepare patients and collect recording clinical information as per protocol.
• Educate patients regarding illness and self-management.
• Carry out standing orders for refills and tests.
• Document all patient interactions in the EHR.
• Conduct outreach to patients for appointments and test results.
• Answer and route patient messages using the EHR.
• Monitor task buckets and referrals; follow up on overdue items.
• Support Clinical Lead; precept new staff.
