RN - PATIENT CARE COORDINATOR

Located at the Togus, ME location. This position is full-time at 40 hours per week. NOTE: The 2-page Resume requirement does not apply to this position.

Responsibilities

• Coordinate and oversee patient care operations, ensuring the nursing needs of patients are met throughout the continuum of care.
• Serve as a nursing administrator and supervisor, providing leadership and direction to nursing staff during assigned shifts, including off-tour and holiday coverage.
• Facilitate patient flow, admissions, transfers, and discharges to promote efficient utilization of resources and timely patient care.
• Assist the Nurse Manager in the management and coordination of nursing services, ensuring safe, effective, and high-quality care delivery.
• Provide leadership and guidance to interdisciplinary teams by serving as a clinical expert, role model, and resource for staff.
• Direct nursing staff and coordinate patient care activities across all areas of Patient Care Services during off tours and holidays.
• Monitor regulatory compliance and identify operational issues, implementing corrective actions and service recovery measures when needed.
• Ensure accurate and timely completion of staffing and scheduling documentation to support efficient workforce management.
• Collaborate with healthcare professionals and stakeholders to develop, implement, and evaluate patient-centered plans of care.
• Lead quality improvement, patient safety, and evidence-based practice initiatives to enhance clinical outcomes, patient satisfaction, and operational effectiveness.
