Job summary

Jackson Hospital is seeking a Nurse Care Coordinator (RN) to support medical plans, educate patients and families, and coordinate care with physicians and clinic staff. This role includes nursing tasks, vital signs, specimen collection, EKGs and assisting with minor procedures as ordered.

Requires Alabama RN licensure, CPR certification, and strong communication skills. A BSN is preferred; ACLS is a plus. Excellent documentation and teamwork are essential for this role.

Qualifications

• Registered Nurse with Alabama license or eligibility.
• Baccalaureate degree preferred.
• Current CPR certification required; ACLS preferred.
• Experience in physician office settings preferred.

Responsibilities

• Coordinate patient care and family education to support care plans.
• Interact with physicians, clinic staff, and patients to ensure quality care.
• Perform routine nursing tasks, vitals, and specimen collection as ordered.
• Assist with front office duties and referrals documentation where needed.
• Maintain Meaningful Use compliant electronic medical record documentation.
