Job summary

Pleasant Hill Post Acute is seeking a compassionate Registered Nurse (RN) to join our team on a Part-Time / On-Call basis. The ideal candidate will provide resident-centered care, collaborate with residents, families, physicians, and the interdisciplinary healthcare team to ensure high-quality outcomes.

Requirements include a California RN license and prior skilled nursing or post-acute experience. We offer competitive pay at $45.00–$50.00 per hour, flexible scheduling, and a supportive work

Responsibilities

• Provide skilled nursing care to residents per individualized care plans.
• Administer medications and treatments as ordered.

• Perform nursing assessments and assist with care planning.
• Maintain accurate and timely clinical documentation.
• Communicate with residents, families, physicians, and staff.
• Follow infection control, safety, and facility policies.

Responsibilities

• Provide skilled nursing care to residents in accordance with individualized care plans.
• Administer medications and treatments as ordered.
