Job summary

Pleasant Hill Post Acute is seeking a compassionate Registered Nurse (RN) to join our weekend and on-call nursing team. You will provide skilled nursing care, administer medications, monitor residents, and collaborate with physicians and staff to maintain high-quality, resident-centered care.

The ideal candidate holds a California RN license, has strong clinical and communication skills, and can work independently in a fast-paced setting while maintaining safety and documentation standards.

Qualifications

• Current and valid California Registered Nurse (RN) license.
• Previous skilled nursing or post-acute experience preferred.
• Strong clinical, communication, and organizational skills.
• Reliable, compassionate, and team-oriented.
• Ability to work independently and effectively in a fast-paced healthcare environment.

Responsibilities

• Provide skilled nursing care to residents in accordance with individualized care plans.
• Administer medications and treatments as ordered.
• Monitor residents' conditions and promptly report changes in health status.
• Perform nursing assessments and assist with care planning.
• Maintain accurate and timely clinical documentation.
• Communicate effectively with residents, families, physicians, and healthcare staff.
• Follow infection control, safety, and facility policies and procedures.
• Promote resident dignity, comfort, safety, and overall well-being.
• Respond appropriately to changes in residents' conditions and emergency situations.
