Job summary

Behavioral Health Network (BHN) is seeking a Registered Nurse to join the PACT team to deliver direct and indirect care, coordinate assessments, and support recovery in Western Massachusetts.

The RN will follow MAP policies, manage medications with physician supervision, and document care in CareLogic and Smartsheet. A Bachelor’s degree in nursing and an active RN license are required; strong collaboration and communication are essential.

Qualifications

• Bachelor’s degree in nursing required.
• Current, unrestricted Registered Nurse license.
• Six months of experience in human services preferred.
• Knowledge of MAP requirements and medication management.
• Ability to work with diverse populations and strong communication skills.

Responsibilities

• Provide direct and indirect nursing services to individuals served by BHN's PACT team.
• Coordinate and complete physical-health assessments and referrals; integrate medical and psychiatric treatment.
• Follow MAP policies and document health- and medication-related information.
• Manage medications and clinical workflows under supervision of the team psychiatrist.
• Develop, implement, and document treatment plans and recovery goals.
• Serve as primary case manager for assigned caseload as needed.
• Maintain shift logs, ensure safety and OSHA compliance, and report concerns.
• Engage individuals using motivational interviewing and promote community integration.
• Provide safe transportation for medical appointments and other service needs.
• Maintain timely documentation in CareLogic, Smartsheet, and other systems.
