Job summary

Miami Jewish Health in Florida is seeking an RN for the PACE program to provide nursing care under supervision of the clinic manager RN and medical director.

Requirements include an accredited nursing degree, an active Florida RN license, BLS, and experience with frail elderly populations. Bilingual English/Spanish is a plus. We offer competitive compensation and comprehensive benefits.

Qualifications

• Graduate from an accredited school of nursing.
• At least 1 year of RN experience or 3 years of LPN experience.
• Minimum one-year experience working with a frail/elderly population.
• Current Florida RN license; BLS certification; valid FL driver’s license & proof of insurance.

Responsibilities

• Performs comprehensive nursing history and physical on new referrals.
• Creates appropriate care plans and participates in IDT.
• Evaluates and triages participants with episodic illnesses.
• Refers participants to medical director or medical specialist as indicated.
• Active participation in family conferences and interdisciplinary team meetings.
