Job summary

Miami Jewish Health in Fort Lauderdale, FL, seeks a Nurse Practitioner for the Florida PACE program to provide direct primary medical care and counseling to enrolled participants. The NP will perform histories and exams, manage acute and chronic illnesses, coordinate with medical directors and specialists, and participate in family conferences and team meetings.

Applicants must be an ARNP or PA with Florida licensure, BLS, and experience with frail elderly; ACLS preferred.

Qualifications

• Graduate from an accredited school of nursing
• Advanced ARNP or PA certification
• Current ARNP/PA license (FL)
• BLS certification
• DEA license required

Responsibilities

• Performs comprehensive history and physical on new referrals and quarterly follow-ups.
• Manages acute, episodic, and chronic illnesses.
• Admits patients and coordinates hospital care and discharge planning with the medical director.
• Coordinates care with medical directors and specialists as needed.
• Documents treatment in patient charts per established guidelines and participates in team meetings and family conferences.
