Job summary

Aviata Health Group is seeking an experienced MDS Coordinator (RN or LPN) to join our Palm Harbor facility. The role centers on leading MDS, CAAs and Care Plans in alignment with federal and state standards.

Qualified candidates hold a current RN/LPN license and have MDS coordination experience. Six months in this role is preferred; RAC-CT or RNAC credentials are a plus. We offer a supportive team environment and strong benefits.

Qualifications

• MDS experience is required.
• Must possess a current, unencumbered, active state license to practice as an RN or LPN.
• Six (6) months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.
