Job summary

Aviata Health Group is seeking a qualified RN or LPN MDS Coordinator to join our team in Tampa, Florida. As an MDS Coordinator, you will oversee the coordination and completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations.

Responsibilities include overseeing MDS coordination, monitoring Medicare and Managed Care residents with the facility team, and driving process improvements to ensure high-quality resident care.

Qualifications

• Must possess a current, active state license to practice as an RN or LPN.
• Six months of experience as an MDS Coordinator.
• RAC-CT or RNAC certification preferred.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Oversee the coordination and completion of the MDS, CAA’s and Care Plan in accordance with Federal and State regulations.
• Monitor and document the management of Medicare and Managed Care residents with facility team members.
• Contribute to process improvements and new ideas to continually improve systems and care delivery.

Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Requirements

• Must possess a current, unencumbered, active state license to practice as an RN or LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six (6) months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
