Job summary

Aviata Health Group in Tallahassee, FL is seeking an RN or LPN MDS Coordinator to oversee the Resident Assessment Instrument process and ensure federal/state regulatory compliance in coordination with the facility team.

The role includes monitoring Medicare/Managed Care resident plans, contributing to process improvements, and maintaining compassionate, patient-centered care. Must hold current RN or LPN license; SNF experience and MDS coordination background preferred.

Qualifications

• Must possess a current, unencumbered license to practice as an LPN or RN.
• Experience as a MDS Coordinator is beneficial.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• RAC-CT or RNAC certification is preferred.

Responsibilities

• Oversee the coordination and completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) per Federal and State regulations.
• Monitor and document Medicare and Managed Care resident management in collaboration with facility staff.
• Contribute to process improvements and innovative ideas to continually enhance systems and outcomes.

Major Responsibilities

• Oversee the coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA’s and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.

Qualifications

• Must possess a current, unencumbered, active state license to practice as an LPN or RN
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six (6) months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• You must be qualified, compassionate, and dedicated to a job well done.
