Job summary

Aviata Health Group is seeking an RN or LPN MDS Coordinator to join our family-oriented team in Florida. You will oversee MDS, CAAs, and Care Plans while ensuring compliance with Federal and State regulations.

Ideal candidates have SNF/Rehabilitation facility exposure and at least six months in MDS coordination; RAC-CT or RNAC is preferred. We value compassion, dedication, and high standards of care.

Qualifications

• Current, unencumbered, active state license to practice as an RN or LPN.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.
• Six months of experience as a MDS Coordinator.
• RAC-CT or RNAC preferred.
• Compassionate and dedicated to a job well done.

Responsibilities

• Oversee coordination and participate in the completion of the Resident Assessment Instrument (MDS, CAA's and Care Plan) in accordance with current Federal and State Regulations.
• Monitor and document the management of the Medicare and Managed Care residents in collaboration with facility team members.
• Provide innovative, responsible healthcare with the creation and implementation of new ideas and concepts that continually improve systems and processes to achieve superior results.
