Qualifications

• MDS experience required and lead MDS responsibilities.
• Active RN or LPN license to practice.
• Experience in Skilled Nursing/Rehabilitation facilities preferred.

Responsibilities

• Oversee coordination and completion of MDS/CARs/Care Plans per regulations.
• Monitor Medicare/Managed Care residents with facility team.
• Develop and implement ideas to improve care processes and systems.
• MDS experience is required.
