Job summary

Fairlawn Haven in Archbold, OH seeks a Licensed Practical Nurse (LPN) for 12-hour shifts within the Care Center. Responsibilities include monitoring health status across resident care plans and supporting Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods.

You will assist and lead STNA staff, requiring good assessment skills, teamwork, and a positive attitude. Applicants should be a graduate of an accredited nursing program with an active LPN license in good standing.

Qualifications

Full Time or Part Time Opportunities

1st, 2nd, and 3rd Shifts Available

12-hour shifts

Includes holiday and weekend rotation.

An LPN will focus on monitoring health status of resident care plans throughout the Care Center. Assisting our Rehab, Assisted Living, Special Care, and the Long-Term Care neighborhoods. Candidate will assist and lead STNA staff. Ideal candidate should have good assessment skills, the ability to work effectively in a changing environment, be a team player, possess a positive attitude, and be self-motivated.

Qualifications and/or Skills Required:

• Graduate of an accredited Nursing Program;
• Current LPN nursing license, in good standing with the State licensing board;
• Minimum of one (1) year Long Term Care experience, preferred;

• Must be able to communicate effectively with personnel, residents, family members, visitors, government agencies/personnel and the general public;
• Must have excellent attendance and work history;
