Job summary

Fairlawn Haven in Archbold, OH is seeking an RN/LPN for a full-time day shift (1st shift, 7AM - 7:30PM) with holiday and weekend rotation. The role focuses on monitoring health status and resident care plans across the Care Center, Rehab, Assisted Living, Special Care and Long-Term Care neighborhoods.

The ideal candidate will have good assessment skills, be able to work in a changing environment, and be a team player with a positive attitude.

Qualifications

• Graduate of an accredited Nursing Program.
• Current LPN nursing license in good standing.
• Minimum of one year Long Term Care experience preferred.
• Ability to maintain confidentiality and Resident’s Rights.
• Effective communication with personnel, residents, family members and the public.
• Excellent attendance and work history.

Responsibilities

• RN/LPN will monitor health status of resident care plans across the Care Center.
• Assist our Rehab, Assisted Living, Special Care, and Long-Term Care neighborhoods.
• Assist and lead STNA staff.
• Must have strong assessment skills and be able to work in a changing environment.
• Be a team player with a positive attitude and be self-motivated.
