Job summary

Anoka Rehabilitation & Living Center, part of Volunteers of America National Services, seeks a LPN or RN to provide resident care under RN supervision. Responsibilities include charting, care plan updates, and collaboration with the care team to ensure safety and quality of care.

Qualifications

• Active licensure as a LPN or RN in MN.
• Effective communicator with staff, families and outside agencies.
• Ability to prioritize duties.
• Customer service oriented with knowledge of successful customer service techniques.
• Ability to understand residents’ charts, doctor’s orders, residents’ plan of care, medication orders.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.

Responsibilities

• Charting and documentation of resident care plans and notes.
• Provide monitoring and attend nurse and team meetings.
• Address non-delegated tasks and report to RN as needed.
