Job summary

The Rochester Rehabilitation & Living Center invites qualified LPNs and RNs to join our on-site nursing team. This full-time day-shift role, 6:30am–7:00pm, supports resident care under supervision, ensuring safety, timely medication administration and comprehensive care planning at our Rochester campus.

Qualifications

• Active licensure as a MN LPN or RN.
• Effective communicator with staff, families and outside agencies.
• Ability to prioritize duties.
• Customer service oriented with knowledge of customer service techniques.
• Ability to understand residents’ charts, doctor’s orders, residents’ plan of care, medication orders.
• Ability to gather clinical data in regards to resident status.
• Ability to utilize computers for timekeeping and documentation.

Responsibilities

• Resident Care – support training of care providers, report abuse or neglect, complete admissions and ongoing assessments, pass medications and treatments as needed.
• Charting and Documentation – update charts and care plans, transcript physicians notes or orders.
• Provide Monitoring – model positive communication, observe and report, attend nurse and team meetings.
• Available to take calls for non-delegated tasks and report to RN.

Shift: 6:30am-7:00pm FT Days
Week 1: T/W/TH
Week 2: W/TH
Week 3 : Sa/Su/M
Select other shifts to get additional overtime
Prefer at minimum 2 years of SNF/TCU experience

LPN Salary : $34.00- $39.00

About the Job:

The Licensed Practical Nurse (LPN) OR Registered Nurse (RN) shall perform in accordance with state and local laws and within the guidelines of his/her professional organization. All functions shall be performed in accordance with the established policies and practices of Agency and the particular institution in which the LPN/RN is performing his/her duties. The LPN/RN is qualified by education, training, experience and demonstrated abilities to give nursing care under the direction of a RN and to have the knowledge of the principles of psychological, biological and social sciences. Resident Care – Support training of all care providers, immediate report any suspected abuse or neglect, complete admission and ongoing assessments, pass medications and complete treatments as needed. Ensure awareness and educate on DNR status.

- Charting and Documentation – Ensure residents charts and care plans are updated. Accurately transcript physicians notes or orders.

- Provide Monitoring – Role model for caregiver, provide positive communication, observe and listen to report, attend nurse and team meetings, Confirm personal correctly assigned based on census and care needs.

- Available to take calls for non-delegated tasks that should be reported to RN. Understating what is to report to RN.

Required Qualifications:

• Active licensure as a LPN OR RN in the state of MN.
• Effective communicator with staff, families and outside agencies.
• Ability to prioritize duties.
• Customer service oriented with knowledge of successful customer service techniques.
• Ability to understand residents’ charts, doctor’s orders, residents’ plan of care, medication orders.
• Must possess the ability to effectively gather clinical data in regards to resident status and condition.
• Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
