Job summary

Qualifications

• RN license required; Florida-based practice preferred.
• BLS certificiation required.
• Oncology experience or Oncology Certification preferred.
• Experience with Epic or similar EMR is a plus.
• Bilingual English/Spanish preferred.

Responsibilities

• Act as liaison across multidisciplinary team to ensure proper orders, medical records, consents and other required documentation.
• Coordinate timely care with physicians and supportive services departments.
• Maintain professional relationships with patients, families and team members.
• Provide direct patient care coordination within Community Care and Transitional Care Programs.
