Southcoast Health System, Inc. in Massachusetts seeks an RN-Practice Navigator-Oncology to coordinate care for cancer patients, from diagnosis through survivorship.

This role focuses on education, resource referrals, and collaboration with the cancer center team to ensure seamless patient experiences. The position offers a 32-hour day shift at the Fairhaven Cancer Center, with competitive pay and a comprehensive benefits package.

Qualifications

• Strong oncology background required.
• RNlicensure in Massachusetts required.
• BSN or higher degree required.
• OCN certification preferred; willing to obtain within 12 months.
• Must be fully vaccinated against influenza and COVID-19 or eligible for exemption with signed statement.

Responsibilities

• Provide consistent care coordination across the continuum, assessing physical, psychological and social needs.
• Serve as ongoing resource for patients from initial diagnosis through treatment and survivorship.
• Organize and facilitate clinical visit needs; educate patients and families and refer to resources.
• Support patient and families to ensure adherence with care plans and access to support services.
• Develop collaborative relationships internally and externally; multi-task in a fast-paced oncology setting.
