Job summary

Southcoast Health is seeking an RN-Practice Navigator-Oncology at the Cancer Center in Fall River, MA. You will coordinate care across the cancer care continuum, assess patient needs, and serve as a resource for diagnosis, treatment, and survivorship.

Responsibilities include organizing visit needs, educating patients and families, referring to support services, and fostering collaboration with care teams to ensure seamless access to resources and adherence to care plans.

Qualifications

• Strong oncology background required.
• OCN certification or willingness to obtain within 1 year of hire.
• BSN or equivalent clinical experience required.
• Oncology patient navigator training preferred.
• Knowledge of insurance reimbursement systems and financial hardship considerations.
• Excellent critical thinking and interpersonal skills.
• Ability to build collaborative internal and external relationships.
• Ability to multi-task in a fast-paced oncology setting.
• Knowledge of community resources for patients and families.
• Over 3 years of clinical oncology experience in outpatient or inpatient settings.

Responsibilities

• Coordinate care across the cancer care continuum and assess patient needs.
• Serve as an ongoing resource for patients from diagnosis through survivorship.
• Organize and facilitate clinical visit needs and referrals to support services.
• Educate patients and families and provide resource information.
• Support adherence to care plans and connect patients with community resources.
