Southcoast Health is hiring an RN-Care Navigator for the OB/GYN department in New Bedford, MA. The role involves care coordination across the continuum and supporting patients with education and resources. The position is day shift, 8:00 am to 4:30 pm, with 24 flex 32 hours, offering a comprehensive benefits package.

Qualified candidates will have a BSN, MA/RI RN licensure, BLS, and experience in ambulatory or related settings. Vaccination or approved exemption is required.

Qualifications

• Graduate of an accredited School of Nursing or equivalent is required.
• BSN in Nursing or equivalent education and experience required.
• Current RN Licensure in the Commonwealth of Massachusetts and/or Rhode Island is required.
• Basic Life Support (BLS) certification is required.
• Oncology and/or GYN experience preferred.
• Must be fully vaccinated against seasonal Influenza and COVID-19 or have an approved exemption.

Responsabilités

• Provide consistent care coordination across the continuum, assessing the physical, psychological and social needs of the patient.
• Serve as ongoing resource for patients throughout the care continuum.
• Organize and facilitate clinical visit needs and education for patients and families.
• Create, monitor, and improve clinical quality indicators for program evaluation and regulatory requirements.
