Job summary

Hartford HealthCare Medical Group is seeking a registered nurse to provide skilled nursing care in a variety of clinical settings under provider direction. The role involves assessing needs, planning and evaluating care, triaging calls, and supporting patients and families with empathy and confidentiality.

The position reports to a Practice Manager. Qualified candidates will hold a Connecticut RN license and have experience in a medical/surgical office setting; familiarity with medical software

Qualifications

• Connecticut RN license required.
• Experience in medical/surgical office setting preferred.
• Experience with medical software or EMR systems.

Responsibilities

• Provide office-based nursing care in collaboration with provider.
• Assess physical and psychosocial needs and plan, implement, and evaluate care.
• Triages telephone calls and initiates appropriate nursing interventions.
• Demonstrate independent nursing actions based on assessment and problem identification.
• Ensure continuity of patient care through documentation and follow-up.
