Job summary

Responsibilities

• Performs and documents patient assessments, planning, interventions and evaluations.
• Administers medications safely according to the five rights and monitors patient response.
• Determines and revises the plan of care with patient/family involvement.
• Develops an education plan and assesses learning needs of patient/family.
• Uses EMR and computerized systems for documentation of care and daily tasks.
• Maintains confidentiality and addresses psychological, social, cultural, and spiritual needs.
