Qualifications

• RN licensure in the state of practice.
• Associate's Degree in Nursing required.
• Typically requires 3 years of clinical nursing experience.

Responsibilities

• Educates and serves as a resource for employers and injured employees, interpreting care plans and medical results.
• Assess patient history, current conditions, and response to treatment to guide a care plan for recovery and return to work.
• Coordinate timely, evidence-based care across the care continuum and ensure smooth transitions.
• Support patients and families with education, psychosocial needs, advocacy, and barrier identification.
• Collaborate with providers to expedite care and adjust plans as needed.
• Facilitate communication among patient/family, physicians, nurses, dietitians, rehab, and other disciplines.
• Identify and resolve barriers to timely discharge.
• Collect data on outcomes and utilization for reporting to committees.
• Work with medical staff and departments to expedite care delivery and documentation.
• Promote professional relationships and problem-solving among staff.
• Provide age-appropriate care, applying growth and development principles.
